Private assessment request

Start with the practical details you already know.

Share enough for a coordinator to understand the administrative request—not your full medical history. You stay in control of what happens next, and licensed physicians remain responsible for every medical decision.

Assessment request

Tell a coordinator what you are exploring.

Three short steps. We usually reply within one business day. This form has no file upload and nothing submitted here is treated as a diagnosis.

Step 1 of 3Where & what

Let's start with the basics

Two quick questions so we can route your request to the right coordinator.

Prefer a conversation?

Choose a coordinator call when you are ready.

Open the live calendar only when you want to book. The call covers administrative coordination and next steps; it is not a medical consultation.

Booking is momentarily unavailable

Our scheduler isn't reachable right now. Send us a message and a coordinator will arrange a time in your timezone.

Use the assessment form
After you submit

A clear handoff, not an automatic medical pathway.

Submission starts an administrative review. It does not create a patient–doctor relationship, reserve treatment, or authorise the transfer of complete medical records.

  1. A coordinator checks the request

We review the practical scope and reply with any administrative questions needed to understand the case.

  2. Records move only after agreement

The first form does not upload medical files. A coordinator explains the secure records handoff if you choose to continue.

  3. The hospital retains clinical authority

Licensed hospital physicians decide whether to review or accept a case and make every diagnosis or treatment recommendation.
